Arrangement Forms


The forms listed below must be completed to make final arrangements for someone who has passed away.  The legal next of kin defined by CA Health & Safety Code 7100   must sign all forms.  If you are not signing the documents in our presence all signatures must be notarized.   If you have questions regarding the proper completion of these forms please call our office and one our personnel will assist you.  


Cremation Forms: 1, 2. 3. 4. 5 and 6

Funeral/Burial Forms: 1, 2, 3 and 6


Note: Form 7 will only need to be completed if the deceased is at the Sacramento County Coroners Office
